Supported Housing…

is for single homeless people who have been assessed by our staff as unable to sustain their tenancy without ongoing support. The aims of this scheme are to:

• work with service users in agreed areas in order to enable them to sustain their tenancy

• enable service users to develop the skills for independent living

• assist in obtaining appropriate move-on accommodation and the transition between supported housing and independent living

Our supported housing service covers Broadland, Breckland, South Norfolk and Babergh Districts.

We accept referrals from these local authorities as well as directly from social services, probation, prisons and other voluntary agencies. Self-referrals are also considered, but we do require information from relevant agencies to support applications. In order to be accepted for supported housing, applicants must meet our admission criteria for each scheme. These are available upon application. Men and women aged eighteen upwards can apply to us. Most of our accommodation is either one or two bedroom flats where we provide low to medium visiting support. We also have a small hostel for men with high support needs in Diss. All of the accommodation has basic furnishings. With the exception of our hostel, all service users are provided with assured shorthold tenancies.

We also have a small number of Floating Support packages. These are for single people who live in their own accommodation in Norfolk, who require support to help them sustain their tenancies.

The referral process is initiated by an agency acting on behalf of the referral, or by people referring themselves contacting us by phone. An application form will then be sent out by us to the agency or the person self-referring. We also ask for any relevant information including reports to be sent with the returned application. Self-referrals will need to provide a named person with whom they have had contact, who knows about their personal circumstances, whom we can approach for further information. Once the application form and supporting information is returned we then assess whether the applicant is suitable and if so arrange an interview. This can either be at our offices or if this is not possible we will travel to the applicant. If, after interview the applicant meets the criteria for the scheme, but there are no vacancies, they will be informed in writing that their application has been placed on the waiting list. If we reject an application we will write stating the reasons. If we accept an applicant then we will arrange for a suitable date for the move in to the property.

The service

• We will assess every application and give a written response stating why applicants have been rejected or accepted

• If successful with the application but there are no available properties, applicants will be placed on a waiting list

• Once accepted into our accommodation, we will undertake an assessment of needs with the service users and agree a support plan with them

• We will help service users claim the benefits they are entitled to

• We will meet with service users on an individual basis at least every two weeks to discuss issues and to help them work towards greater independence

• We will help service users to put their own ideas for the future into action

• We will provide accommodation and furnishings of a reasonable standard

• We will assist in obtaining move-on accommodation
